ConferMED, a network of eConsultants for primary care providers (PCPs) in Federally Qualified Health Centers (FGHCs), can help you with peer-to-peer consultation. It is available at no cost through 12/31/2023. Interested FQHCs and PCPs will have access to ConferMED’s endocrinology specialists to assist with optimal management of patients with diabetes. Particular focus is placed on recommending CGMs and DSME to appropriate patients.
How to Get Started With ConferMED
- Use your population health and UDS data to identify patients with diabetes for whom you would like an eConsult from an expert endocrinologist.
- Access your secure eConsult submission portal by clicking on the unique link provided to you by ConferMED.
