MyCare Member ID Card (Opt-in/Opt-Out)
Opt-In/Opt-Out Ohio MyCare member ID cards have:
- MCP logo
- MyCare Ohio logo
Opt-In (MyCare Ohio Medicare+Medicaid) member ID cards have:
- Medicare Rx logo
- CMS number at the bottom of card
Opt-Out (MyCare Ohio Medicaid only) member ID cards have:
- No Medicare Rx logo
Medicaid Member ID Card
Ohio Medicaid member ID cards have:
- Next Gen logo (state image with banner)
- Gainwell Pharmacy Benefit information
- OhioRise Aetna logo if member is enrolled in OhioRISE
